Lab, Inc. and Federal Express have reached an agreement
that our doctors can ship packages to our Lab from anywhere
in the United States. By using, “Our Airbills,”
the rate per package is $12.90 when marked Economy Two Day
and the package weighs one pound.
By calling the Federal Express phone number 1-800-GO-FedEx
(1-800-463-3339) and giving them your name, address and
zip, a driver by means of an onboard computer, is dispatched
to pick up your package. “Our Airbills” which
are enclosed, should be completed prior to the arrival of
the Federal Express driver.
Below are instructions to fill out “Our Pre-printed
Fedex Airbill.”
Only these sections have to be filled out. Leave the rest
of the form blank.
Section 1: Is to be filled in with your office
information.
Section 2: Optional
Section 3- 4: Pre-typed (Lab. name & address, FedEx
Two-Day)**
Section 5-6: Leave blank
Section 7: Fill in the Total Packages and the Total Weight.
Generally, 3 to 4 cases can be put into a package marked
one pound.
If the weight of the package is over two pounds, mark that
weight. Make sure you mark the weight in pounds only.
**If you use Priority Overnight, Standard Overnight or
Saturday service, we will bill you for any extra charges.
When the driver arrives, ask him to use the “FedEx
Pak Envelope” and ask him to give you extras.
On arrival at our Lab, the package is opened and the number
of Rx’s is noted and marked.
With, a pro-rated delivery cost, for example: 1 Rx = $12.90,
2 Rx’s = $6.45 each,
3 Rx’s = $4.30 each, and so on. That will be your
delivery cost per Rx.
Dockstader pays the return “Air-bill!”
Shipped in the “FedEx Pak Envelope” and marked
“Economy Two-Day” on the “Envelope”,
the package has an 80% chance of being delivered overnight
and guaranteed delivery the 2nd day.
