Dockstader Orthodontic Symposium is the sole owner of the information collected on this site. We will not sell, share, or rent this information to other companies or individuals.

Contact Us

As an option we provide on-line forms for users to contact various areas of our organization. In order to contact us via the Internet a user is required to give their contact information (such as name and email address). This information is used to contact the user regarding a specific request or about the services on our site for which they have expressed interest. It is optional for the user to provide other information such address and phone contact.

Information Request Forms

On some Dockstader Orthodontic Symposium web sites, as an option, we provide on-line forms for users to register to receive product information, techniques, promotions, or other publications via e-mail and/or direct mail.

Our users are given the opportunity to ‘opt-out’ of having their information used for purposes not directly related to our site at the point where we ask for the information.

The Dockstader Orthodontic Laboratory is designated an approved PACE Program Provider by the California Academy of General Dentistry. The formal continuing education programs of this program provider are accepted by AGD for Fellowship, Mastership and membership maintenance credit. Approval does not imply acceptance by a state or provincial board of dentistry. The current term of approval extends from October 1, 2004 to October 1, 2007.
