Information Collection and Use
Dockstader Orthodontic Symposium is the sole owner of the information
collected on this site. We will not sell, share, or rent this information
to other companies or individuals.
Contact Us
As an option we provide on-line forms for users to contact various
areas of our organization. In order to contact us via the Internet
a user is required to give their contact information (such as name
and email address). This information is used to contact the user
regarding a specific request or about the services on our site for
which they have expressed interest. It is optional for the user
to provide other information such address and phone contact.
Information Request Forms
On some Dockstader Orthodontic Symposium web sites, as an option,
we provide on-line forms for users to register to receive product
information, techniques, promotions, or other publications via e-mail
and/or direct mail.
This information is not sent out in an unsolicited manner, and is
only sent to users that have expressed an interest in receiving
the information. To unsubscribe from e-mail distribution lists,
you can send an email to info@dockstader.com.
All personal information submitted to us via information request
forms is used only to send users the promotional material and is
only accessible by Dockstader Orthodontic Symposium employees and
contractors involved in the fulfillment of the information.

On some Dockstader Orthodontic Symposium web sites we may use cookies.

on are kept in a secure environment.

Correction/Updating Personal Information
If a user’s personally identifiable information changes (such
as your zip code), or if a user no longer desires our service, we
will correct, update or remove that user’s personal data provided

Choice/Opt-out
Our users are given the opportunity to ‘opt-out’ of
having their information used for purposes not directly related
to our site at the point where we ask for the information.
For example, certain customer care request forms allow the user
to note that their question or request should be kept private and
not included in a frequently asked question area.
Notification of Changes
If we decide to change our privacy policy, we will post those changes
on this page so our users are always aware of what information we
collect, how we use it, and under circumstances, if any, we disclose
it. If at any point we decide to use personally identifiable information
in a manner different from that stated at the time it was collected,

in this different manner. We will use information in accordance

The Dockstader Orthodontic Laboratory is designated
an approved PACE Program Provider by the California Academy
of General Dentistry. The formal continuing education programs
of this program provider are accepted by AGD for Fellowship,
Mastership and membership maintenance credit. Approval does
not imply acceptance by a state or provincial board of dentistry.
The current term of approval extends from October 1, 2004
to October 1, 2007. |
