Cruise Reservation Policy:
A deposit of $250 per person is required to reserve
your room. Final payment is due Dec 1, 2010.
Not included: Air transportation, transfers, items of personal
nature, such as shore excursions, specialty restaurant fees,
some beverages, photographs, gratuities/services fees, medical
services, etc.
Cancellation Policy:
In addition to a $50/person Lewis Travel administrative
fee. the following cancellation penalties apply with Royal
Caribbean Cruise Liner (RCCL):
67 t o 30 days before departure - $250 per person 12/14/2010
29 to 8 days before departure - 50% per guest 01/21/2011
7 to 0 days before departure - 100% per guest 02/12/2011
We recommend the purchase of travel insurance. Call Paulette

RCCL reserves the right
to impose a fuel surcharge on all guests if the price of
West Texas fuel exceeds $65 per barrel.

jaw orthopedics for correction of Class II malocclusions. He
emphasized the patient’s face, full smile, jaw joints,

With John’s passing much enthusiasm has been lost and
the fundamentals of FJO are lost for most general dentists and

Education Credits (Dental Board of California and also
for the Academy of General Dentistry).
The Dockstader Orthodontic Laboratory is designated an
approved PACE Program Provider by the California Academy
of General Dentistry. The formal continuing education
programs of this program provider are accepted by AGD
for Fellowship, Mastership and membership maintenance
credit. Approval does not imply acceptance by a state
or provincial board of dentistry. The current term of
approval extends from October 1, 2007 to October 1, 2010.
