Cruise Reservation Policy:

A deposit of 20% per person is required to reserve your suite. Final payment is due March 20, 2008.

Cancellation Policy:

In addition to a $50/person cancellation administrative fee. the following cancellation penalties apply:

120 to 91 days before departure - $200 per person

90 to 60 days before departure - 10%

59 to 30 days before departure - 25%

29 to 15 days departure - 50%

Less than 15 days before departure - 100%

Dr. David Salmassy

Dr. David A. Salmassy, DMD

Dr. David Salmassy is a graduate of Oregon Health Sciences University in Portland, Oregon. His residency training in Oral & Maxillofacial Surgery was completed at the Univ. of CA, San Francisco. He is a member of the American Associated of Oral & Maxillogacial Surgeons, and the International Congress of Oral Implantologists. He maintains a private practice in Auburn, CA.

Robert J. Matkovich, DDS

Dr. Robert Matkovich graduated from Georgetown University School of Dentistry in 1977 with Honors. He completed his General Practice Residency at the Veteran’s Administration Hospital in La Jolla, CA. Dr. Matkovich served as the Chief of Dental Services at Hillside Hospital in San Diego for 10 years and is currently in private practice in San Diego, CA.

Dr. Matkovich is a Diplomate of the American Board of Special Care Dentistry (ABSCD), a Fellow of the American Association of Hospital Dentists (AAHD) and a Fellow & Founding Member of the World Congress of Minimally Invasive Dentistry (WCMID). He is a member of the AACD, IAO, AAFO and ALD.

The Dockstader Orthodontic Laboratory is designated an approved PACE Program Provider by the California Academy of General Dentistry. The formal continuing education programs of this program provider are accepted by AGD for Fellowship, Mastership and membership maintenance credit. Approval does not imply acceptance by a state or provincial board of dentistry. The current term of approval extends from October 1, 2007 to October 1, 2010.
