Policy
Billing
& Payment
Invoices: An
invoice is generated for each prescribed appliance (or service)
you request for an individual patient. The invoice is dated on the
day of completion and is included with the appliance on delivery
to your office. Keep the invoice to cross check the monthly statement.
Monthly Statement: The monthly statement is a summary of
the financial transactions that have occurred with your account
in the current month. Your statement is printed and dated at the
closing of the last working day of the month.
Payment Terms: Net 15 Days: The current
charges on the monthly statement are due and payable by the 15th
of the month in which you receive the statement.
Past Due Accounts: All payments are due and payable by the
15th of the month in which you receive your statement. Payments
on statement balance not received by the 20th of the month will
be charged a $15.00 late fee that will appear on your next statement.
Credit cannot be extended beyond the last working day of the month
in which you receive your statement.
Credit card payments:
We accept all major credit card payments.
Our Guarantee
Dockstader guarantees that our appliances will fit the
models on which we fabricate the appliances. We recommend immediate
pouring of alginate impressions and that the Doctor, check all models
before sending them to the lab. Models should be wrapped individually
and be secured tightly in the shipping box or bag.
Repairs
Cost for repairs or remakes are determined by appliance
age, type of fractures and model distortion (if any). Original working
models and/or new models should be returned along with the original
appliance.
Pick up & Delivery
Doctors can use prepaid mailers, California Overnight,
UPS, or Federal Express. We recommend prepaid mailers, California
Overnight and UPS in California. For WA, OR, and NV, we recommend
prepaid mailers and UPS.
In all other states, Pick up & Delivery is by Federal Express
because it is the most reliable and quickest courier we have found
for long distance service. The cost can be as low as $2.18 per case.
